Clinical Care Coordination

Case management services, which we refer to as care coordination, are provided in a culturally responsive, client centered manner. Our priority is to ensure that services are coordinated and implemented based on the client's most pressing needs.  After completing a comprehensive assessment with our clients, we incorporate an active engagement strategy to directly link clients to community resources and support.  We recognize that clients who are dealing with the aftermath of acute trauma require more than a traditional referral.  CITRC care coordination services are delivered utilizing holistic therapeutic interventions that are integrated into the client's individualized treatment plan. Through this integrated team approach to care, services are better coordinated, thus allowing us to treat the whole person as they recover and heal.
